GRU's health sciences campus has immunization requirements in addition to the USG Board of Regents requirements. Students need to be fully immunized to prevent the spread or contracting of contagious diseases.
To attend classes per the Board of Regents, the University System of Georgia, and the GRU immunization policy, all health sciences students (including on-line and distance learning) must meet the established immunization requirements.
Please print the following form: Certificate of Immunization. Complete this form in English and fax it to 706-434-7417, along with the required titer results 90 days prior to start of class. This form must be signed by the student and a certified health care provider.
Please go to STUDENT HEALTH ON-LINE at https://osh.gru.edu. Use your Outlook user name and password to complete the required Medical History form. Click on the Medical History link, complete the form and submit.
